Step One
Fill out the form below or call the office to schedule your no- cost consultation. We will find a time that works best for your schedule!
Step Two
Once here, we will have you fill out a health history form and give you a tour of the office.
Step Three
One of our doctors will sit down with you to perform a thorough consultation and health history at no charge. The purpose is to see if we are a good fit for your needs, so the consultation is just a conversation and not a commitment. If everything makes sense and you choose to move forward with the examination, it will be performed that day.
